The Cornell Scott-Hill Health Center’s (CS-HHC) mission is to measurably improve the health and well-being of the communities we serve by providing excellent and compassionate care, accessible to all. CS-HHC provides medical, dental, behavioral and enabling services.The Fellow will use population health management software to improve patient outcomes, reduce patient costs, and realize greater patient satisfaction. The Fellow will generate/analyze data; develop care delivery strategies; and work with the Chiefs of Medicine and Operations and others to evaluate data and facilitate the institutionalization of the work.

Fair Haven Community Health Center

The mission of the Fair Haven Community Health Center (FHCHC) is to provide comprehensive primary health care, delivered through innovative and alternative systems of health delivery at a reduced costs for those most vulnerable in our community.  FHCHC serves patients within the New Haven community and nearby areas; services include Adult medicine, Pediatrics, Women’s Health, Behavioral Health Services, Dentistry and various specialties. The Fellow will  help review clinical work flows, policies and procedures, update content with evidence-based resources, reconcile stored documents and special projects such as assisting in the design and implementation of medical, vaccine, pharmaceutical supply inventory tracking systems, and other administrative tasks related to clinical operations.

Sickle Cell Disease Association of America, Southern CT

The mission of Sickle Cell Disease Association of America, Southern Connecticut Inc. (SCDAA) is to provide education, screening, counseling, advocacy and support services to persons affected with sickle cell disease and the trait.  Its goal is to ensure a network of partners dedicated to promoting a healthy lifestyle for patients with sickle cell disease.  The Fellow will conduct outreach education, be a community advocate, help develop and refine a social media-based information vehicle that is youth-focused.   The Fellow will also promote awareness and prevention of sickle cell disease by targeting increased numbers of youth and young adults of child-bearing age.
